The Pet Owner agrees to the following terms and conditions:

1.    The Pet Owner will provide all required leashes, harnesses,  jackets collars,  food, (or any other items deemed  required for the pets service)

 

2.    The Pet Owner agrees to provide Vany's 4 Paw Paradise Pet Services with all necessary instructions for their pets requirements while under their care.

 

3.    The Pet Owner will provide any medication or special needs instructions for each of the  pets.

 

4.    The Pet Owner will be considered liable for any damages or injuries caused by the pets while under the care of Vany's 4 Paw Paradise pet Services.

 

5.     The Pet Owner will pay Van's 4 Paw Paradise pet Services  50% for all fees and services one week prior to the start date of service(s)

by cash or e‐transfer.  vany.nanny@gmail.com

 

6.    Vany's 4 paw paradise pet  Services will perform the duties listed by the Pet Owner to the best of {his/her} abilities, in a reliable and caring manner.

 

7.    Van's 4 Paw paradise pet Services will keep secure and confidential any keys, pass codes, access numbers and personal information, and return them

at the end of the contracted service date.

 

8.     Vany's 4 paw paradise Pet Services will not be liable for any injuries to the dogs that are caused by animals or people outside of Van's 4 paw paradise  Pet Services control.

 

9.    Vany's 4 paw paradise Pet Services will not be liable for any incident that occurs during transportation of the dogs to a vet, kennel or clinic. 

Pet Requirements:

 

I represent that my pet meets all of the following requirements: 

 is current on his/her required vaccinations,

 is on a monthly flea and tick preventative medication,

 has been in good health for the last 30 days

 my pet is not aggressive or toy protective, 

 I have completed the Dog or Cat Profile

 

1. HEALTH . I represent that my pet has not had any contagious illnesses of any kind for 30 days prior to check‐in. I am aware and understand that Vany's 4 Paw Paradise Pet Services employees are not veterinarians and do not have backgrounds in animal medicine. Vany's 4 Paw Pet Services employees cannot diagnose  or detect illnesses in the pets that are staying at Van's 4 Paw Paradise Pet Services located at 47 Rang de la riviere Noir S J0S 1R0 Saint-Chrysostome Quebec. I agree to assume all risk

associated with the administration of medication by Vany's 4 Paw Paradise Pet Services during my pet’s stay. In addition, I acknowledge and am aware that vaccines do not protect against all contagious illnesses that may affect my pet. I HEREBY AGREE TO INDEMNIFY VANY'S 4 PAW PARADISE PET SERVICES, ITS OWNERS, DIRECTORS, OFFICERS, EMPLOYEES AND AGENTS AGAINST ANY CLAIMS MADE AGAINST  VANY'S 4 PAW PARADISE PET SERVICES, ITS OWNERS, DIRECTORS, OFFICERS,

EMPLOYEES, AND AGENTS AS A RESULT OF MY FAILURE TO INFORM  VANY'S 4 PAW PARADISE PET SERVICES  OF ANY PRE‐EXISTING MEDICAL CONDITIONS THAT MY PET MAY HAVE

 

2. Flea and Ticks. I agree that if any fleas or ticks are discovered on my pet during check‐in or at any other time while my pet is receiving services from

 VANY'S 4 PAW PARADISE Pet Services  may administer a flea bath and flea spot treatment at my expense. 

 

3. Veterinarian Liability and Care. I agree to allow  VANY'S 4 PAW PARADISE Pet Services to obtain veterinarian medical treatment for my pet, if, in its sole discretion it appears that, the pet is ill, injured, or exhibits any other behavior that would reasonably suggest that my pet might need medical treatment. Medical treatment may require transportation of my pet to receive care and I hereby authorize such transportation. I grant  VANY'S 4 PAW PARADISE Pet Services  full authority to make decisions involving the medical treatment of my pet during its stay at  VANY'S 4 PAW PARADISE Pet Services . I agree that I am fully responsible for the cost of any such medical treatment and transportation. I agree that I am assuming all risk of illness, disease, harm or otherwise to my pet by allowing my pet to

participate in services at  VANY'S 4 PAW PARADISE Pet Services . Furthermore, I agree that I am assuming all risk of the consequences associated with any decisions

made by  VANY'S 4 PAW PARADISE Pet Services , relating to the medical care and transportation of my pet. 

 

4. Transportation. I agree that if my pet is transported to or from  VANY'S 4 PAW PARADISE Pet Services by  VANY'S 4 PAW PARADISE Pet Services, its employees or agents that I

AGREE TO HOLD  VANY'S 4 PAW PARADISE PET SERVICES, ITS OWNERS, DIRECTORS, OFFICERS, EMPLOYEES, OR AGENTS HARMLESS IN THE EVENT OF INJURY OR

ACCIDENT DURING TRANSPORTATION. 

 

5. Pet Behavior. I agree to be solely financially responsible for any and all acts or behavior of my pet while in the care of VANY'S 4 PAW PARADISE Pet Services, WHICH

MAY INCLUDE PAYMENT OF DAMAGES FOR THE INJURY CAUSED BY MY PET TO OTHER ANIMALS, HUMANS, AND/OR VANY'S 4 PAW PARADISE PET SERVICES. IN

ADDITION, I AGREE THAT IF MY PET IS INJURED BY ANOTHER PET, I HEREBY RELEASE VANY'S 4 PAW PARADISE PET SERVICES, ITS OWNERS, EMPLOYEES, AND AGENTS

FROM ALL LIABILTY AND FINANCIAL RESPONSIBILITY FOR SUCH INJURY. I FURTHER UNDERSTAND THAT IF MY PET BITES A HUMAN OR PET, THAT VANY'S 4 PAW PARADISE PET SERVICES MAY CONTACT THE APPROPRIATE AUTHORITIES. 

 

6. Dog Daycare. I understand that VANY'S 4 PAW PARADISE Pet Services is a cage‐free facility utilizing playgroups where multiple dogs interact. I understand that dogs

play with their mouth and paws, which can result in nicks and scratches on my pet. While VANY'S 4 PAW PARADISE Pet Services provides reasonable care and

supervision in the playgroups, I understand and agree that VANY'S 4 PAW PARADISE Pet Services employees may not notice these nicks or scratches before my pet’s departure and, therefore, I might not be notified. 

 

7. Aggressive Dogs. I certify that my dog is not aggressive and I understand that aggressive dogs are not permitted to participate in services at VANY'S 4 PAW PARADISE Pet Services. If my dog acts aggressively or exhibits unacceptable behavior, he/she may be separated from the other dogs. I authorize VANY'S 4 PAW PARADISE Pet Services to use squirt bottles, and in extreme cases muzzles to control my pet for the protection of other pet guests and humans.

 

8. Abandonment Notice. I fully understand and agree that if my pet is not picked up by myself or an authorized representative within 5 calendar days after the day my pet is scheduled to depart, that my pet shall be deemed “abandoned”. I understand if I abandon my pet at VANY'S 4 PAW PARADISE Pet Services, VANY'S 4 PAW PARADISE  Pet Services, in its sole discretion, will try to re‐home my pet, or relinquish my pet to a legal shelter of its choice. I FULLY UNDERSTAND AND AGREE THAT IF I ABANDON MY PET AT VANY'S 4 PAW PARADISE PET SERVICES, 47 RANG DE LA RIVIERE NOIR S J0S 1R0 SAINT-CHRYSOSTOME  I MAY BE UNABLE TO RETRIEVE MY PET AND WILL HAVE NO RECOURSE AGAINST VANY'S 4 PAW PARADISE  PET SERVICES. In addition, I understand that I will still be responsible for the unpaid charges incurred for my pet’s stay.

 

9. Photo and Video Release. I agree to allow VANY'S 4 PAW PARADISE Pet Services to use my pet’s name and any images or videos taken while he/she is in the care of VANY'S 4 PAW PARADISE Pet Services, in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials.  

 

10. Personal Property. I agree that VANY'S 4 PAW PARADISE Pet Services shall not be responsible or liable for any lost, stolen, or damaged personal property belonging either to my dog or me. I also understand and agree that my dog’s collar may  be removed in the play area to prevent injury. 

 

11. Service Fees. I agree to pay 50% for all fees and services prior to my pets arrival with cash, or by e-transfer.  The remainder will be paid by cash at the time of my pet’s pickup from each visit at  VANY'S 4 PAW PARADISE Pet Service.

12. Reservations. I understand that confirmed reservations are required for boarding services at VANY'S 4 PAW PARADISE Pet Services. I understand there is a two (2) night minimum for all boarding reservations during any Peak/Holiday periods.  

 

13. Cancellations. Non holiday/non‐ peak policy: I understand that all confirmed reservations for NON HOLIDAY/NON PEAK DAYS must be cancelled at least seven (7) days prior to my pet’s reserved arrival date. I further understand that a cancellation made within seven (7) days of my pet’s arrival date or a no- show will result in charges for two (2) night’s stay. HOLIDAY/PEAK DAY POLICY: I understand that all confirmed reservations for HOLIDAY/PEAK DAYS must be cancelled at least fifteen (15) days prior to my pet’s reserved arrival date. I further understand that cancellation made within fifteen (15) days of my pet’s reserved arrival date, or a no show, will result in no refund.

 

14. Check-in & Check-out Times. I understand that checking in before 7:00 a.m. incurs an additional service fee of $15.00. Check-out time is from 2:00 p.m. I understand checking my pet out after 6:00 p.m. will be charged an additional  service fee of $20.00 .
